Job Summary
The Home Health Quality Assurance (QA) Coordinator is responsible for monitoring, reviewing, and improving the quality of clinical documentation and services provided by Sora Home Health. This role ensures compliance with Medicare, state, and accreditation standards, including CMS and ACHC guidelines. The QA Coordinator works closely with the Director of Nursing (DON), clinical staff, and administrative team to maintain high standards of patient care and regulatory compliance.
Key ResponsibilitiesClinical Documentation Review
- Review OASIS assessments for accuracy, completeness, and compliance with CMS guidelines
- Audit start-of-care (SOC), resumption of care (ROC), recertification, and discharge documentation
- Ensure physician orders, plans of care, and visit notes meet regulatory requirements
- Verify documentation supports medical necessity and homebound status
Compliance & Regulatory Oversight
- Ensure agency compliance with Medicare Conditions of Participation (CoPs)
- Monitor adherence to ACHC accreditation standards
- Identify and correct documentation deficiencies before billing
- Assist with preparation for surveys, audits, and ADR requests
Quality Improvement
- Participate in agency QAPI (Quality Assurance Performance Improvement) program
- Track quality indicators and clinical outcomes
- Develop corrective action plans for identified deficiencies
- Assist in implementing best practices to improve patient outcomes
Staff Support & Education
- Provide guidance to clinicians regarding documentation requirements
- Educate staff on OASIS accuracy and compliance standards
- Collaborate with the Director of Nursing on performance improvement initiatives
Chart Audits
- Conduct routine chart audits to ensure compliance with:
- Plan of care
- Visit frequency
- Timely documentation
- Physician orders
- OASIS submission timelines
QualificationsRequired
- Registered Nurse (RN) or Licensed Vocational Nurse (LVN) license in the State of California
- Minimum 2 years of Home Health experience
- Strong knowledge of OASIS documentation
- Understanding of Medicare Conditions of Participation
- Experience with home health EMR systems (Axxess, WellSky, or similar)
Preferred
- Experience in Home Health QA or Clinical Management
- Familiarity with ACHC accreditation standards
- Knowledge of QAPI programs

Employment Type

Pay: $22.00 - $30.00 per hour
