We are seeking a highly detail-oriented and experienced Home Health Quality Assurance (QA) Specialist to join our team. This role is essential in ensuring compliance with state, federal, and accreditation standards while supporting the delivery of high-quality patient care.
Key Responsibilities:
- Review and audit clinical documentation, including OASIS assessments, plans of care, and visit notes, for accuracy, completeness, and regulatory compliance
- Ensure adherence to Medicare, state, and agency guidelines
- Collaborate with clinicians to provide feedback and education on documentation best practices
- Monitor quality indicators and identify trends for performance improvement
- Assist with preparation for surveys, audits, and accreditation reviews
- Support ongoing quality improvement initiatives and corrective action plans
Qualifications:
- Licensed nurse (RN or LVN) preferred
- Prior experience in home health QA, clinical review, or case management
- Strong knowledge of OASIS, Medicare Conditions of Participation, and home health regulations
- Exceptional attention to detail and organizational skills
- Ability to work independently and meet deadlines in a fast-paced environment
