Job Summary
The Home Health Case Coordinator supports the clinical and administrative operations of Sora Home Health by coordinating patient care services, assisting with scheduling, tracking physician orders, and ensuring timely communication between clinicians, physicians, and office staff. The Case Coordinator helps ensure patient care is delivered efficiently while maintaining compliance with Medicare and agency policies.
Key ResponsibilitiesPatient Care Coordination
- Coordinate patient care services between clinicians, physicians, and office staff
- Assist with scheduling nursing, therapy, and home health aide visits
- Monitor visit frequency and ensure patients receive ordered services
- Communicate patient status updates with the Director of Nursing (DON) and clinical team
Physician Orders & Documentation
- Track physician orders and ensure timely signatures from physicians
- Assist with obtaining Face-to-Face documentation and medical records from MD offices
- Monitor Plan of Care (POC) updates and recertifications
- Ensure documentation is received and uploaded into the EMR system
Intake & Referral Support
- Assist with processing new patient referrals
- Verify patient demographics, insurance information, and physician orders
- Coordinate start of care visits with clinicians
- Communicate with referral sources regarding patient status
Scheduling
- Assist with clinician scheduling and visit assignments
- Adjust schedules as needed based on patient needs and clinician availability
- Monitor missed visits and reschedule when necessary
Communication
- Serve as a liaison between physicians, patients, clinicians, and administrative staff
- Respond to inquiries from patients and referral sources
- Ensure timely communication regarding changes in patient condition or orders
QualificationsRequired
- Minimum 1–2 years experience in Home Health or healthcare coordination
- Strong organizational and communication skills
- Ability to manage multiple tasks and prioritize workflow
- Experience with EMR systems (Axxess, WellSky, Data Soft Logic, etc.)
Preferred
- Knowledge of home health Medicare guidelines
- Experience with scheduling and intake coordination
- Understanding of Plan of Care and physician orders

Pay: $25.00 - $30.00 per hour
