We are currently looking for a Home Health Care Coordinator for our Covina branch! LVN Preferred but we are open to non-licensed candidates if you have the right experience.
The hourly range for this position at commencement of employment is expected to be between $33-35 for LVN's and $28-30 for non-clinical applicants; however, pay offered may vary depending on multiple individualized factors, including job-related knowledge, skills, and experience.

What You'll Do
Keep our home health team running smoothly — managing schedules, coordinating patient care, and making sure nothing falls through the cracks. You'll be the hub connecting patients, clinical staff, physicians, and insurance payors to deliver seamless care.
Day-to-Day Responsibilities
Primarily office-based with some walking, standing, and occasional off-site visits. Possible exposure to communicable diseases.

Home Health Care Coordinator
What You'll Do

Day-to-Day Responsibilities
- Manage scheduling for all home care services and nursing on-call rotations
- Handle intake referrals and follow up on patient concerns
- Communicate with physicians to receive updated orders
- Request and manage authorizations with third-party payors
- Participate in weekly case conferences and team meetings
- Support field staff with nursing care functions as needed
- Uphold patient rights, privacy, and confidentiality at all times
- Licensed Vocational Nurse (*Preferred but not mandatory) licensed by the California Board of Vocational Nurse and Psychiatric Technician Examiners
- Organized, reliable, and cool under pressure
- Strong communicator — written and verbal
- Professional and people-oriented in every interaction
- Committed to quality care and continuous improvement
- Valid auto insurance and a safe driving record required
