Description:Access TLC is seeking a compassionate and motivated ECM Care Coordinator to join our San Fernando team.As an ECM Care Coordinator, you will provide primarily in-office case management services while conducting regular field visits to patients in the community. You will play a key role in coordinating care, connecting patients to community-based services, and ensuring timely and efficient delivery of support.Access TLC supports patients by arranging community-based and supportive services that enhance quality of life and promote independence. We are looking for someone who is eager to learn, compassionate, dependable, and a true team player.Compensation$20.00/hour – Without a Bachelor’s degree$22.00/hour – With a Bachelor’s degreePosition ResponsibilitiesConduct patient assessments and develop individualized care plansCoordinate and monitor service deliveryMaintain detailed progress notes and case recordsConduct quality assurance reviews and collect required program dataPerform field visits to establish ongoing eligibilityMonitor patient safety and ensure services identified in the Individual Service Plan are implementedAddress patient concerns and advocate for needed resourcesRequirements:RequirementsBachelor’s or Master’s degree in one of the following (preferred but not required for $20 rate): Social Work, Psychology, Counseling, Rehabilitation, Gerontology, Sociology, Health or Human Services.Clean California Driver’s LicenseReliable vehicle with insurance meeting state and/or organizational requirementsFluent in Spanish and English (preffered)Strong organizational, communication, and documentation skills
