Solutions Medical Center is seeking a motivated, professional, and compassionate Patient Care Coordinator to join our growing functional medicine practice. This role serves as the first point of contact for our patients and plays a critical role in creating a welcoming, organized, and supportive experience throughout their care journey.
The ideal candidate combines excellent customer service skills with strong administrative abilities and clear communication. You will help guide patients through appointments, services, and next steps while supporting clinic operations and team workflow.
This position is essential to ensuring a smooth, seamless, and elevated patient experience from check-in through follow-up.
Responsibilities
- Greet and welcome patients warmly, professionally, and with empathy
- Schedule, confirm, and coordinate appointments while managing patient flow efficiently
- Answer phone calls, texts, and emails promptly and accurately
- Maintain accurate and confidential patient records within the electronic health record system
- Assist patients with forms, intake paperwork, and clinic policies
- Address patient questions and concerns with professionalism and discretion
- Educate patients on clinic services, treatment plans, memberships, and available products
- Support retail and supplement coordination in alignment with provider recommendations
- Assist medical staff with administrative support, including chart preparation and documentation
- Ensure patient documentation is complete, accurate, and up to date
- Communicate clearly with patients regarding follow-up care and next steps
- Collaborate closely with providers and clinical staff to ensure continuity of care
Qualifications
- Bachelor’s degree in healthcare, nutrition, wellness, or a related field preferred
- 1–2 years of experience in a medical, wellness, or patient-facing clinical role preferred
- Interest in or understanding of functional or integrative medicine (preferred, not required)
- Strong ability to explain care processes and clinic services clearly to patients
- Excellent verbal and written communication skills
- High attention to detail and strong organizational skills
- Ability to multitask, prioritize effectively, and work in a fast-paced environment
- Professional demeanor with a strong focus on patient experience and satisfaction
- Sales or retail experience is a plus but not required
- Team-oriented mindset with a positive, proactive attitude
