We are seeking a dedicated and skilled Wound Care Nurse Practitioner to join our
healthcare team. In this role, you will be responsible for providing specialized care to
patients with complex wound management needs. You will work collaboratively with a
multidisciplinary team in a medical office setting, ensuring optimal patient outcomes
through comprehensive assessment, treatment planning, and education.
Responsibilities
Conduct thorough assessments of patients with wounds, including history taking
and physical examinations.
Develop and implement individualized care plans based on patient needs and
evidence-based practices.
Perform wound care procedures, including dressing changes, debridement, and
management of infection.
Collaborate with physicians and other healthcare professionals for effective
discharge planning and utilization management.
Maintain accurate medical documentation in compliance with regulatory
standards and coding requirements.
Utilize eClinicalWorks for patient records management and documentation.
Educate patients and their families on wound care techniques, nutrition, and
overall health maintenance.
Participate in quality improvement initiatives related to wound care services
within the facility.
Qualifications
Valid Nurse Practitioner license with specialization in wound care or related field.
Experience in a medical office setting is preferred; familiarity with teaching
hospitals is a plus.
Proficient in medical documentation practices and coding related to wound care.
Skills In Venipuncture And Other Clinical Procedures Are Advantageous.
Strong understanding of discharge planning processes and utilization
management principles.
Ability to lift patients as needed while ensuring safety protocols are followed.
Excellent communication skills to effectively educate patients and collaborate
with the healthcare team.

Pay: $94 - $102 per hour
