The role is based in Fremont, CA, at a 256-bed skilled nursing facility featuring three skilled nursing stations, a sub-acute unit, and a locked memory care unit. This setting offers a collaborative environment where speech therapists work closely with physical and occupational therapists, as well as nursing staff, to deliver comprehensive rehabilitative services.
Key Responsibilities:
- Deliver skilled speech-language pathology services to residents across skilled nursing, sub-acute, and memory care units.
- Collaborate with an interdisciplinary team of therapists and nurses to develop and implement individualized treatment plans.
- Utilize electronic medical records (Nethealth and PCC) to document patient progress and coordinate care effectively.
- Provide mentoring and support to therapists new to skilled nursing or unfamiliar with facility-specific EMR systems.
- Maintain compliance with clinical and regulatory standards.
