Home Staff Pharmacist | Brooklyn, New York

A healthcare facility in Brooklyn, NY is seeking two experienced pharmacists for a 13-week contract with a potential transition to permanent employment based on performance. This role requires strong expertise in home infusion pharmacy and IV certification to support patient care and medication management.

Key Responsibilities

• Accept and dispense prescription medications accurately.
• Monitor patient medication adherence and identify over-usage patterns.
• Ensure compliance with legal regulations regarding medication supply.
• Verify the suitability of prescribed medications for each patient.
• Maintain confidential and precise patient records and pharmacy files.
• Counsel patients on medication usage, potential side effects, and answer their questions.
• Provide additional patient services, including smoking cessation guidance, blood pressure, and cholesterol monitoring.
• Educate patients on the proper handling of medical devices such as blood pressure monitors and glucose meters.
• Compound medications safely for internal or external use.
• Prepare sterile IV solutions following established protocols.
• Supervise pharmacy staff including technicians and clerks.
• Maintain adequate inventory of pharmaceutical and medical supplies.
• Collaborate with healthcare professionals to address drug-related issues and optimize patient outcomes.
  
  

Qualifications And Experience

• Doctor of Pharmacy degree (PharmD) required.
• Valid New York State pharmacy license in-hand.
• Prior IV pharmacy experience mandatory, preferably with home infusion expertise.
• Strong communication and interpersonal skills for patient and team interaction.
  
  

Schedule

• Monday through Friday, 8:30 AM to 5:00 PM.
