Seeking an experienced Infusion Pharmacist for a rewarding opportunity supporting a maternity leave assignment. Responsibilities

• Oversee and verify home infusion medication orders to ensure accuracy and safety
• Provide clinical support and guidance on infusion therapy protocols and patient care plans
• Collaborate with interdisciplinary teams to optimize pharmacotherapy outcomes
• Monitor patient progress and adjust therapy as needed under established guidelines

Qualifications

• Licensed Pharmacist with current state licensure in Connecticut
• Proven experience in home infusion pharmacy and clinical pharmacy practice
• Strong knowledge of infusion protocols, sterile compounding, and medication management

• Monday through Friday, 8:00 AM to 5:00 PM
