Schedule: Variable days and afternoons, primarily day shifts with 8-hour shifts

Responsibilities

• Provide clinical pharmacy services with emphasis on ambulatory care and population health management
• Collaborate with healthcare teams to optimize medication use in patients, focusing on chronic disease states like diabetes
• Perform staffing duties as part of daily operations, ensuring medication safety and effective pharmacy workflow
• Support the growth and development of clinical pharmacy programs within the health system

Qualifications & Desired Experience

• Licensed Pharmacist in Pennsylvania
• Experience in ambulatory care or inpatient pharmacy preferred
• Completion of residency program highly desirable
• Strong clinical knowledge and passion for patient-centered care
