JOB SUMMARY: The Post Acute Liaison is responsible for developing and maintaining strategic referral relationships to support census growth for Solheim Senior Community’s Skilled Nursing and Rehabilitation Center. This role serves as Solheim’s primary external representative to hospitals, physicians, discharge planners, case managers, managed care organizations, and community healthcare partners.
The position focuses on post-acute business development, referral management, market analysis, transition coordination, and strategic outreach initiatives designed to strengthen Solheim’s presence within the healthcare continuum. The role also collaborates with interdisciplinary departments to facilitate efficient admissions and smooth transitions of care for incoming residents.
This position exercises independent judgment in managing referral pipelines, evaluating referral opportunities, coordinating external partnerships, tracking census and market trends, and implementing outreach strategies aligned with organizational goals.
Occasional weekends and community events may be required.
ESSENTIAL JOB FUNCTIONS:
- Develops and maintains strategic relationships with hospitals, physicians, discharge planners, case managers, managed care organizations, accountable care organizations (ACOs), and community healthcare partners to promote Solheim’s post-acute services.
- Serves as Solheim’s primary liaison for post-acute referral development and external healthcare networking.
- Identifies opportunities to increase census, referral volume, and market presence through targeted outreach and relationship-building activities.
- Conducts regular visits, presentations, and follow-up communications with referral sources to strengthen partnerships and maintain visibility within the healthcare community.
- Participates in industry networking groups, healthcare meetings, and community outreach events representing Solheim.
- Monitors referral trends, market conditions, hospital discharge patterns, and competitive landscape to identify growth opportunities.
- Tracks and analyzes admissions metrics, conversion rates, referral outcomes, and census development data; prepares reports and recommendations for leadership.
- Assists leadership in developing and implementing strategic marketing and post-acute growth initiatives.
- Collaborates with leadership to establish census development goals and referral strategies.
- Reviews and evaluates incoming referrals for clinical appropriateness, payer source compatibility, bed availability, and service capability.
- Coordinates with nursing leadership, therapy, social services, admissions support staff, and interdisciplinary departments to facilitate efficient admissions and transitions of care.
- Communicates effectively with residents, families, and healthcare professionals regarding available services, admission criteria, payer sources, and post-acute care options.
- Conducts professional facility tours and provides education regarding Solheim’s rehabilitation and skilled nursing services.
- Coordinates assigned post-acute initiatives and special projects including planning, implementation timelines, deliverables, reporting, and follow-up activities.
- Identifies operational barriers impacting admissions, referrals, or transition processes and recommends process improvement solutions.
- Supports quality improvement efforts related to admissions, referral management, customer satisfaction, and transition coordination.
- Maintains accurate referral logs, outreach activity documentation, market data, and communication records.
- Ensures compliance with HIPAA, Medicare, Medi-Cal, managed care requirements, and all applicable federal, state, and local regulations.
- Maintains confidentiality of resident and organizational information.
- Maintains regular attendance and professionalism in all interactions.
- Performs other duties as assigned.
EDUCATION/EXPERIENCE:
- Bachelor’s degree in Healthcare Administration, Marketing, Business, Nursing, Social Work, or
related field preferred.
- Minimum of three (3) years’ experience in: post-acute care, skilled nursing business development,
healthcare marketing, managed care, discharge planning, or related healthcare setting preferred.
- Experience working with hospitals, case managers, and referral networks is strongly preferred.
SKILLS/KNOWLEDGE/ABILITY:
- Strong knowledge of: skilled nursing operations, Medicare, Medi-Cal, managed care, post-acute services, and discharge planning processes.
- Demonstrated business development and relationship management skills.
- Ability to independently manage outreach strategies and referral pipelines.
- Ability to analyze admissions trends, referral data, and operational metrics.
- Excellent verbal and written communication skills.

Salary : $80,000 - $88,000
