Summary

SOF Missions is a faith-based non-profit organization that provides suicide prevention services and holistic medical care to veterans and military members. We are seeking a highly organized and detail-oriented Case Manager to join our growing team.

The Case Manager assigned to Veterans Services will support the mission through intake, screening, assessment, referral, and linkages to services via telephone, email, IM/Chat, text messaging, and in-person interactions. The Case Manager also supports the Be Resilient Clinics through logistical and operational support.


Duties and Responsibilities

  • Counsel veterans to facilitate achieving developed care plan goals while sustaining wellness and resiliency.
  • Assist the Clinic Manager with the coordination and execution of Be Resilient Clinics, including managing veterans and health care providers.
  • Handle veteran applications, conduct interviews, and prepare notes for committee approval.
  • Compose correspondence, case notes, narrative and technical reports, notifications, and related documents.
  • Work with committee members to assist in the development of care plans for veterans.
  • Track progress of veterans through maintaining a current log of veteran status.
  • Utilize surveys and/or other tools to gather statistical data to reflect the efficacy of SOF Missions veteran programs.
  • Research programs and applicable resources for veterans.
  • Communicate with public, volunteers, and supporters via email, phone, and in person.
  • Pack and ship veteran resources as needed.

Education and Experience

·      Minimum bachelor’s degree (or currently pursuing) in social work, psychology, nursing, or a closely related field is preferred.

·      U.S. veteran of a branch of the United States Military (honorably discharged, retired, or reservist) preferred.

·      Experience working with the veteran population highly desirable.

·      At least two (2) years of experience in social casework, counseling, healthcare coordination, eligibility determination, social services, or community resource referral.

·      CAN or LPN certification highly desired with active state licensure as an LVN/LPN in Florida.


Knowledge, Skills, and Abilities

  • Knowledge of general medical and behavioral health interviewing practices and techniques.
  • Knowledge of social work and counseling practices and techniques, including crisis prevention and intervention.
  • Knowledge of HIPAA regulations and patient confidentiality protocols.
  • Knowledge of Apple based computer systems and Microsoft Office Suite.
  • Knowledge of community resources and veteran healthcare services.
  • Ability to document case records and contacts accurately.
  • Ability to communicate effectively verbally and in writing.
  • Ability to work, problem solve and make decisions independently.
  • Ability to prepare narrative reports related to work, contracts, and grants.


Physical Demands & Working Conditions

Physical Requirements: Requires good hand-eye coordination, arm and hand dexterity, and visual acuity to use a keyboard. Regularly required to stand, walk, sit, and use computer equipment for extended periods. Some lifting and patient assistance may be required.

Working Conditions: Duties are performed primarily in an office setting. Must be able to operate a computer and standard office equipment such as a telephone and copier/printer. Also required to travel in their personal vehicle.

Safety Considerations: Will periodically encounter difficult or complex veterans, as well as volatile individuals. Training in Trauma-Informed Care, Mental Health First Aid, Psychological First Aid, and personal safety will be provided or required.

Travel: Minimal. The work may require driving in Hillsborough County and occasionally outside the county for clinics and fundraising events.


Salary & Work Schedule

💰 Salary: $45,000 - $50,000 (based on experience and educational background).

⏰ Hours: M-F 8:00 am – 3:00 pm. Occasional evenings and weekends required based on clinic schedule.
