What are the responsibilities and job description for the Resident Case Manager position at Social Service Staffing & Recruiting, Inc.?
This is temporary for 1 - 2 months, could be extended. It is 4 - 5 days/ week, M - F 9AM - 5:30PM in person. The Case Manager provides comprehensive, client-centered intake, assessment, planning, coordination and follow-up services for lesbian, gay, bisexual and transgender, and queer (LGBTQ ) seniors and adults with disabilities with multiple housing, health, and psychosocial challenges that impair their functional capacity to access needed services. The Case Manager works with clients and their existing formal and informal support networks to develop care plans and to monitor services and interventions to ensure provision of high-quality care and an optimum level of functioning for the client in the most integrated setting.
The Case Manager assesses needs, develops care plans, authorizes services, arranges and coordinates
the provision of services among providers, provides follow-up monitoring and reassessment, and
develops discharge and termination plans.
The Case Manager will perform the following core functions:
• Identify and assess older adults and adults with disabilities in need of case management
through an intake and enrollment process;
• Assess clients’ functional and cognitive capacity and limitations, including identification of issues
and challenges;
• Develop an individual, client-centered, strengths-based care plan care plan to address the
service needs identified in the assessment;
• Ensure the implementation of the care plan by coordinating and arranging for services as
necessary based on the client’s functional capacity as determined in the assessment;
• Follow up and monitor care plan implementation through quarterly home visits and monthly
(minimum) phone contacts with clients;
• Review and modify care plans on a regular basis to ensure the care plan continues to meet their
needs and the meet quality and effectiveness standards;
• Work with centralized intake system to take on client referrals;
• Counsel, coach, advocate for, and work collaboratively with clients to respond to crisis situations
regarding housing and social service needs;
• Complete a comprehensive reassessment on an annual basis
• Terminate Case Management services when appropriate.
Qualifications:
· BA degree in health, social work, psychology, counseling, gerontology, other related fields plus demonstrated experience working with seniors and/or adults with disabilities OR 4 years experience in case management.
· Experience with and understanding of the housing, medical, and psychosocial issues of lesbian, gay, bisexual, transgender and queer seniors, and adults with disabilities.
· Exceptional communication and presentation skills relating to seniors, adults with disabilities, their caregivers and support systems and teams of health professionals.
