Schedule: Monday–Friday, 9am–5pm
About the Program: Social Clubhouse, Inc. is an adult partial care program providing day services and structured support for adults living with mental health conditions. Social Clubhouse has been serving the adult mental health community in New Jersey for over 20 years. We are expanding our continuum of care to include Outpatient (OP) and Intensive Outpatient (IOP) services, licensed by the New Jersey Division of Mental Health and Addiction Services (DMHAS). These new levels of care aim to provide flexible, client-centered treatment to support recovery and community integration as well as to serve as a step down for our clients in the existing partial care program.
We are affiliated with Social Recovery Center, LLC, also based in Springfield, NJ which offers treatment for substance use and co-occurring mental health recovery.

Position Summary: Case Managers at Social Clubhouse are responsible for service coordination and member advocacy, continuously assessing and referring members on their caseload as needed. They facilitate a daily Home Group in the morning and a Wrap-Up session at the end of the day for their assigned members, and collaborate with Mental Health Specialists on curriculum for other therapeutic groups. Case Managers serve as the primary care coordinators for their caseload, working closely with community providers and the site psychiatric provider to support effective independent living plans.

Key Responsibilities:
- Facilitate groups and create group curriculum in collaboration with other staff members
- Conduct individual sessions with each member on their caseload at least once per week
- Develop person-centered service plans with each member, ensuring goals are assessment-driven and aligned with the member’s clinical needs
- Monitor members’ medical, housing, and overall wellness needs, integrating them into effective service plans
- Coordinate program services in collaboration with Mental Health Specialists
- Perform other duties as assigned by program leadership
- Complete training to provide required group subjects—IMR, Prevocational, and Co-occurring services—and be able to fill in as needed for Mental Health Specialists.
- Bachelor’s degree in a Human Services field, OR
- Associate’s degree with at least two years of experience in human services, OR
- Five years of experience in human services, OR
- Relevant professional credential such as Certified Psychiatric Rehabilitation Practitioner (CPRP), Licensed Associate Counselor (LAC), Bachelor of Social Work (BSW), Certified Rehabilitation Counselor (CRC), Certified Alcohol and Drug Counselor (CADC), Bachelor in Rehabilitation, or Bachelor of Science in Nursing (BSN)
- Paid Time Off (PTO) – up to 80 hours per calendar year
- Paid Sick Leave – up to 40 hours per calendar year

- Up to 3 Personal Days per year
