" Must present current license and be CPR certified (if not certified the hospital can certify them free of charge)
" Use own instruments and products
" Attend an orientation to the hospital (this will be handled by the Rehabilitation Therapy Department)

Question 1
Absences greater than two weeks MUST be approved by CAI management in advance, and contact information must be provided to CAI so that the resource can be reached during his or her absence. The Client has the right to dismiss the resource if he or she does not return to work by the agreed upon date. Do you accept this requirement?

Candidate must attend a 3-day orientation (4 hours for CPR, 1 and � day of Bridge Building, and 1 full day of general orientation) Do you accept this requirement?

Position is for an average of 6 hours per week, Indiana is flexible with the day of the week, hours will be between 9am and 6pm can work 2-3 hours days if candidate prefers.. Do you accept this requirement?

Supplier must be able to support the multi-rate model for this position. Is your payroll team able to process multi-rate payroll?
