About the Opportunity
We are seeking an experienced and detail-oriented Licensed Pharmacist to join a Long-Term Care pharmacy team serving long-term care facilities and assisted living communities.
This is an excellent opportunity for a pharmacist who values medication safety, clinical accuracy, and collaboration with healthcare professionals.
Key Responsibilities
- Review medication orders for appropriate drug selection, dosage, interactions, and administration.
- Verify medications are accurately prepared, dispensed, and labeled.
- Communicate with nursing staff, physicians, and healthcare professionals regarding medication-related concerns.
- Supervise medication preparation and dispensing activities.
- Recommend therapy changes to physicians and facility staff when clinically appropriate.
- Investigate and resolve medication-related questions and issues.
- Maintain accurate patient profiles, including medical history and clinical/laboratory information.
- Ensure compliance with pharmacy procedures, safety standards, and applicable regulations.
Qualifications
- Active Pharmacist license with the Kentucky Board of Pharmacy — Required
- Previous Long-Term Care pharmacy experience preferred
- Strong clinical judgment, analytical, and critical-thinking skills
- Excellent attention to detail and accuracy
- Ability to work independently and efficiently
- Strong communication and multitasking skills
- Computer proficiency required
- FrameworkLTC experience is a plus
- Ability to sit/stand for extended periods and perform light lifting as required
