Summary

We are seeking a dedicated Patient Care Coordinator to join our compassionate team at Skywood Recovery in Augusta, Michigan. This pivotal role involves coordinating patient care and ensuring that each individual receives personalized support throughout their treatment journey, aligning with our commitment to patient-centered care. This position will primarily assist patients with initiating the Short-Term Disability and FMLA paperwork process.
Responsibilities

  • Coordinate patient intake and discharge processes, ensuring seamless transitions.
  • Promptly responds to calls/mail in a professional, courteous, and helpful manner.
  • Formulates Continuing Care Plan with patients.
  • Maintain accurate patient records and documentation in compliance with healthcare regulations.
  • Facilitate Psycho-education groups weekly.
  • Organize and schedule appointments for patients as needed.
  • Facilitates bi-weekly discharge planning groups.
  • Initiates, explains, and completes the Referral and Intake Processes accurately.
  • Assists patients with initiating Short-Term Disability and FMLA paperwork process.
  • Observes all confidentiality regulations.
  • Performs all other duties as required or assigned.

Qualifications

  • Ability to inspire patient confidence in the FRN Staff and the FRN Model of Treatment.
  • Ability to consistently demonstrate through conduct and communications the stability required to effectively assist patients and their family members through the FRN Model of Treatment.
  • Excellent verbal and written communication skills required.
  • Must have high level of interpersonal skills. Position continually requires demonstrated poise, professionalism, tact and diplomacy.
  • Excellent organizational skills with the ability to prioritize workload and meet deadlines.
  • Must have strong knowledge of a variety of computer software applications in word processing, spreadsheets, and presentations (MS Word, Excel, and PowerPoint).
  • Understanding of diverse cultures and gender specific issues and the ability to incorporate needs of gender and culturally diverse groups into the organization.
  • Understands need for and maintains appropriate confidentiality at all times when interacting with patients, residents, families, visitors, referral sources and all other contacts.
  • Exhibits excellent customer relation skills as evidenced by supportive and constructive communication with all contacts including coworkers, patients, residents, visitors, families and referral sources.

Education and Experience

  • Bachelor’s degree in psychology, social work, or related field preferred.
  • Minimum 2 years’ experience in a mental health/healthcare field.

Work Environment and Physical Demands

  • Ability to be physically mobile; climb stairs, traverse the campus on uneven, hilly terrain. However, long periods of sitting will be required.
  • Position requires a great deal of work on the computer requiring manual dexterity and visual acuity.
  • Ability to read, write and analyze detailed and complex information.
  • Must be able to communicate effectively by telephone.
  • A current, valid driver’s license and a clean driving record is required for use of company vehicles as well as occasional business travel.
