The Social Worker collaborates with an interdisciplinary team in the delivery of psychosocial wellbeing needs of the resident, family members and significant others. Promotes a team concept with other departments and disciplines through demonstrating good interpersonal skills.

JOB DESCRIPTION

• Maintains constant interaction with other disciplines in the needs of the residents
• Gather appropriate social information at time of admission
• Participating fully in the care planning process
• In collaboration with interdisciplinary team to ensure continuity of care
• Works with Social Worker Consultant to provide yearly in-service education
• Discharge planning

QUALIFICATIONS

• LSW license required, LICSW preferred
• Bachelor's degree preferred
• 1 yrs. experience in LTC
