Licensed Social Worker
Blaire House of Worcester is a 75-bed skilled nursing center. We have been family owned and operated, servicing the health care needs of seniors for 50 years. We have a unique mission and dedicate 100 percent of our services to Dementia and Alzheimer’s Disease.
Job Description:
- Develop a comprehensive social history.
- Providing social intervention, such as counseling, as patient advocate, as liaison with the community, or regarding financial or emotional issues.
- Maintaining social service progress notes on patient medical records which reflect any changes in patient psycho-social or physical status, evaluation of family interactions, identification of new problem areas, and review of social plans, goals and progress made.
- Develop initial discharge plan and document in clinical record, revising until discharge with available community services.
Qualifications:
EDUCATION: B.A. or M.A. degree in social work or related field. Must be a licensed social worker at the LSW level of licensure or higher.
TRAINING AND EXPERIENCE: At least one year direct social work experience in a long term care facility. Must have a genuine interest in working with geriatric patients and a sincere concern for their welfare and happiness.
Annual Salary: $70,000 -$75,000
