Clinical Hospital Liaison (Hybrid)
Full-Time | Hybrid (2/3 remote)
Pay: $75,000 – $85,000 per year
About the Role:
Are you a connector who thrives on building relationships and helping patients transition to quality care? Our team is seeking a compassionate and dynamic Clinical Hospital Liaison to join our admissions team. This hybrid role includes hospital outreach, virtual collaboration, and in-person support for a seamless transition of patients into our skilled nursing facility.
What You’ll Do:
Build strong relationships with hospital discharge planners, case managers, and physicians
Identify and evaluate potential referrals for skilled nursing services
Serve as the clinical point of contact for hospitals and healthcare partners
Coordinate and streamline admissions by gathering clinical info and ensuring eligibility
Represent the facility with professionalism and compassion
Conduct in-person and virtual assessments of potential residents
Collaborate with nursing, therapy, and administrative teams to ensure smooth patient transitions
Qualifications:
Licensed Practical Nurse (LPN) or Registered Nurse (RN) preferred, or relevant healthcare background
2 years of experience in hospital liaison, marketing, admissions, or care coordination
Experience in long-term care, skilled nursing, or post-acute care
Strong interpersonal, communication, and organizational skills
Familiar with healthcare regulations and insurance verification
Proficient in Microsoft Office and EMR systems
Work Schedule & Flexibility:
Hybrid Schedule: Mix of remote work, in-person hospital visits, and occasional on-site meetings at the nursing facility
