What are the responsibilities and job description for the Admissions Coordinator position at Skilled Nursing and Rehab Care?
Position Overview:
The primary purpose of your position is to coordinate and facilitate referrals for potential admissions. The admissions Coordinator will act as a liaison to the Admissions Director and nursing units to help ensure a smooth transition with the admissions process for all residents. Further, the position will act as a liaison with family members as well as the residents of the Facility. Keep abreast of current Medicare and Medicaid regulations governing admission and discharge requirements of health care facilities.
Key Responsibilities:
- Coordinate all aspects of the admissions process, ensuring compliance with state and federal regulations.
- Maintain accurate admission records and prepare reports as needed.
- Communicate with hospital discharge planners, families, and interdisciplinary team members to ensure a smooth transition for residents.
- Perform data entry and maintain resident information using Visual, Sigma, and Allscripts software.
- Provide excellent customer service to residents, families, and referral sources.
- Collaborate with nursing, social services, and billing departments to ensure a seamless admission process.
- Uphold confidentiality and compliance with DOH regulations at all times.
Qualifications:
- 1–3 years of experience in admissions coordination in a healthcare or skilled nursing facility.
- Proficiency in Visual, Sigma, Allscripts, Microsoft Office, and Outlook.
- Strong knowledge of DOH compliance regulations for admissions.
- Advanced computer and data entry skills.
- Excellent organizational and communication skills.
- High school diploma or equivalent required; degree not required.
- Medicare and Medicaid regulations governing admission and discharge requirements of health care facilities.
Schedule:
Full-time, Monday to Friday, 9 am-5pm
