We are seeking a compassionate and experienced Director of Social Services to lead our Social Services Department in providing high-quality, resident-centered care. The ideal candidate is a strong leader with clinical insight and a passion for enhancing the well-being of our residents.
Key Responsibilities:
Develop and implement departmental policies and procedures in alignment with regulatory requirements.
Recruit, retain, and manage a team of qualified social services professionals.
Uphold and promote the Resident Bill of Rights among residents and staff.
Participate actively in interdisciplinary care planning meetings.
Oversee the quality and effectiveness of social work services, particularly related to discharge planning.
Ensure departmental compliance through the development of written policies and procedures.
Support other departments in meeting regulatory documentation standards.
Prepare Quality Assurance (QA) reports as required.
Serve as the facility’s Grievance Officer, investigating and resolving resident concerns with empathy and professionalism.
Qualifications:
Bachelor’s Degree in Social Work (BSW) or related human services field required
Minimum of 2 years of experience in a healthcare or medical setting
Knowledge of federal and state regulations related to long-term care social services
