Responsibilities
- Develop and implement individualized treatment plans based on thorough assessments of patients’ physiology, anatomy, and medical history
- Conduct patient evaluations and monitor progress across outpatient clinics, inpatient units, home health settings, and rehabilitation centers
- Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to coordinate care plans and discharge strategies
- Manage clinical documentation accurately while maintaining compliance with clinical standards
- Assist patients with therapy techniques such as trigger point therapy, mobility exercises, and other physiotherapy modalities to promote recovery
- Provide patient education on injury prevention, post-treatment care, and community resources for ongoing support
- Participate in discharge planning processes to ensure seamless transition from hospital or outpatient setting to home or community environments
Requirements
- Valid license as a Registered Physical Therapist in the respective jurisdiction
- Proven experience practicing in a physical therapy role within outpatient clinics, inpatient hospitals, or home health settings
- Strong knowledge of physiology, anatomy, medical terminology, and clinical documentation standards
- Experience working with geriatric patients, neurology cases, orthopedics, and outpatient rehabilitation programs
- Familiarity with Kinnser/Wellsky software for accurate documentation and communication
- Ability to develop effective treatment plans tailored to individual patient needs and goals
- Excellent client communication skills to educate patients effectively and foster trust during therapy sessions
