Duties
- Conduct thorough assessments of patients’ physical, emotional, and social needs within their home environment.
- Develop personalized care plans aligned with medical directives and patient preferences, ensuring compliance with healthcare regulations.
- Administer medications, perform wound care, IV, Lab work and monitor vital signs with precision and attention to detail.
- Provide crisis intervention and crisis management when urgent situations arise, ensuring patient safety at all times.
- Educate patients and their families on health management strategies, medication adherence.
- Document all patient interactions accurately using electronic health records, demonstrating strong case management expertise.
- Supervise and mentor junior staff or caregivers as needed to ensure consistent quality of care across the team.
Requirements
- Valid RN (Registered Nurse) license in good standing with current CPR certification.
- Proven experience working with seniors, geriatric patients in a home or community setting.
- Ability to communicate effectively in bilingual settings; fluency in additional languages is a plus.
- Experience working with culturally diverse communities to deliver respectful and inclusive care.
- Demonstrated competency in resource identification within social services to support client needs holistically.
- Familiarity with working with clients who have complex medical conditions
- Excellent interpersonal skills for client communication, motivational interviewing techniques, and collaborating with multidisciplinary teams.
- Experience in IV and Wound Care
- Experience in Kinnser / Wellsky Software
