- Conduct initial patient evaluations and provide an overview of their rehabilitation program.
- Develop individualized treatment plans with the goal of helping patients return home.
- Monitor patient and family progress toward short- and long-term rehab goals, ensuring all necessary care and services are in place for discharge.
- Coordinate Durable Medical Equipment (DME), medical equipment, and follow-up services before discharge.
- Educate patients and caregivers on available follow-up therapy services and make referrals as needed.
- Advocate for patients, ensuring they are actively involved in their rehabilitation journey.
- Ensure the collection of data for PAI documentation and contribute to the discharge planning process.
- Education: Minimum of an Associate's Degree in healthcare or a related field (preferred)
- Experience: At least 1 year of experience in healthcare, ideally in a hospital or rehabilitation setting (inpatient hospital experience preferred)
- Strong communication and interpersonal skills to work effectively with patients, families, and healthcare teams
- Knowledge of rehabilitation processes and discharge planning
- Ability to manage multiple tasks and responsibilities in a fast-paced environment
- A passion for patient care and improving the quality of life for those in recovery
