We are seeking a detail-oriented and tech-savvy Scheduling Specialist to support patient scheduling across both in-home and facility-based care settings. This role is ideal for a Medical Assistant (MA/CMA) or experienced healthcare scheduler who thrives in a fast-paced environment and enjoys coordinating care across multiple teams.

What You’ll Do
- Coordinate and manage daily patient scheduling for providers across multiple locations (SNF, ALF, in-home)
- Add patients to schedules, adjust appointments, and prevent provider overlaps
- Process reschedules, discharges, and patient removals with accuracy and urgency
- Maintain clean, accurate patient lists and scheduling systems
- Collaborate with providers and internal teams to align schedules and patient needs
- Follow provider and facility-specific scheduling protocols
- Manage scheduling requests, ticket queues, voicemails, and email communication
- Ensure proper visit frequency for patient types (Skilled Nursing, ALF, Hospice, Geriatric, Behavioral Health)
- Coordinate telehealth visits and urgent scheduling requests
- Document scheduling updates, visit reasons, and related clinical communication (labs, radiology, etc.)
