Responsibilities
- Deliver comprehensive skilled nursing care to patients within the home setting
- Participates in the development and implementation of patient care plans in collaboration with physicians and the interdisciplinary team
- Provides quality end-of-life cares and support to patient, family, and caregivers
- Flexible, multitasker, demonstrates effective communication and assessment skills
- IA RN license
- BLS required
- Valid driver's license and proof of insurance required
- More career flexibility – create own schedule based on patient caseload for the day
- Greater independence – key communicator between patient, family members, and primary care providers
- Rewarding patient relationships – develop rapport with patients and their family members through case management
- 64 hours a pay period - 8 hour shifts - full-time benefit eligible
- Typically one holiday of on-call only per year - self scheduled
- Typically one weekend of on-call only every 6 weeks - self scheduled
- The schedule may fluctuate based upon the needs of the department
