Job Responsibilities
- Perform regular visits to clients in their homes
- Review care being provided in the homes by our staff
- Ensure home care aides are supervised post-orientation in the home and that skills of the aide match the care needs of the client
- Perform Starts of Care with our new clients in a timely manner
- Manages the Care Plan to be provided by Care staff
- Submit documentation on a timely basis
Key Qualifications
- RN degree
- Solid understanding of home care and field staff disciplines
- Staffing flexibility
- Solid technology skills
- Demonstrated ability to build strong relationships
- High impact communication skills; both written and verbal
- Strategic thinker with strong attention to detail
- Self starter, able to prioritize, meet tight deadlines and manage time effectively
- Demonstrated ability to problem solve and implement solutions collaboratively
- Strong organizational skills
