Registered Nurse

JOB OVERVIEW:

CHC’s Registered Nurse (RN) is a professional member of our health care team who provides skilled nursing care to patients in compliance with the Nurse Practice Act regulations of the State Board of Nursing and within the standards of clinical practices as defined by the American Nurses Association and the policies and procedures of CHC. The RN is responsible for the total nursing care of all assigned patients.

QUALIFICATIONS AND EXPERIENCE:

· Graduate of an accredited school of nursing and must possess a current New Mexico RN license

· A current CPR certification card is required

· Ideally has a minimum of one year of recent experience in an acute care facility as a staff nurse

· Must display good emotional health and be able to physically tolerate much standing, stooping, bending and heavy lifting.

· Must be self directed and able to work with little supervision and have good observational/nursing judgment and communication skills.

· Must meet all of CHC’s requirements for TB health policies and personnel policies.

· Must meet all educational requirements of thirty (30) contact hours for re-licensure every two (2) years.

REPORTS TO: Clinical Services Administrator

DUTIES & RESPONSIBILITIES:

· Makes the initial evaluation/assessment visit

· Develop, evaluate and coordinate the patient’s plan of care on a continuing basis

· Furnishes those services requiring substantial and specialized nursing skill

· Initiates appropriate preventative and rehabilitative nursing procedures

· Evaluates the home for its suitability for the patient’s care

· Regularly re-evaluates the patient’s nursing needs

· Prepares clinical and progress notes, coordinates services, informs the physician and other personnel of changes in the patient’s condition and needs

· Counsels the patient and family in meeting nursing and related needs; teaches the patient and caregivers how to provide care

· Inform the physician and other personnel of changes in the patient’s condition and needs

· Participates in mandatory staff meetings and patient care meetings

· Participates in ongoing education for home health aides, completes skills and performance evaluations

· Attends in-service training on OASIS and best practices as scheduled by the Clinical Services Administrator

· Participates in on-going chart reviews to assure accuracy of clinical records

· Participates in OBQI plan of actions

· Participates in annual evaluation of home health aide performance and skills

· Completes all required clinical paperwork in a timely manner

· Adheres to CHC policies and procedures
