Hospice Intake Director Opportunity Description

Our client is a well-established Home Health organization. They have a current opening for a Home Health Intake Manager for their Home Health location in Berkshire County, MA.  Medicare-certified Home Health experience as a Registered Nurse is required. Must understand intake / admission processes, care coordination, and referral source management. Relocation candidates welcome to apply!
 
Home Health Intake Manager Job Requirements
  • Medicare certified Home Health experience as a Registered Nurse.
  • Intake / Admissions / Care Utilization / Referral Source experience.
  • Supervisory experience strongly preferred.
  • Strong communication and relationship building skills.
Home Health Intake Manager Job Responsibilities
  • Day-to-day oversite of intake department.
  • Coordinate proper placement of patients and appropriate level of care.
  • Network with discharge planners, physicians, referral sources, and patients.
The compensation scale listed is a general salary range and is not intended to represent the actual annual or hourly compensation rate for this role. An offer will be made to the selected candidate at the conclusion of the interview process based on their relevant experience and the responsibilities of the position. Our client does offer a competitive compensation package potentially including incentives, health and dental insurance, PTO, retirement plans (401K) and opportunities for professional development.
