The Silvercrest Center for Nursing and Rehabilitation, proud member of the New York Presbyterian Regional Hospital Network, is a 320-bed skilled nursing located in Briarwood, Queens. We are a unique center of excellence dedicated to providing services to complex acute residents on ventilators, subacute/post-acute care services.

This position will be responsible for providing and overseeing complete resident assessment, care planning, patient and caregiver education, evaluating progress toward goals and will make recommendations for changes. The ideal candidate must have solid experience as a registered nurse, in a long-term care facility, and have working knowledge of electronic medical record.

Duties:
Accepts and distributes all pharmacy and narcotic deliveries
Acts as a role model and resource person in clinical and administrative matters.
Adherence to the standards and mission of the facility as outlined in the Silvercrest Credo.
Assesses the resident’s condition and nursing needs; set goals; prescribes appropriate nursing actions to meet those goals related to the physical, psychological, social and rehabilitative needs of the resident.
Assists the Associate Director of Nursing/Staff Development Coordinator in the orientation program for professional and non-professional nursing staff on the nursing unit.
Assists the Director of Nursing and Associate Director of Nursing in the development and implementation of research studies related to nursing care.
Assists with, or institutes emergency measures for change of condition.
Collaborates with Nursing Administration to ensure that the principles of infection control are maintained.
Collaborates with other members of the health care team as needed to accomplish the plan of care which has been developed based on a comprehensive assessment.
Collaborates with Staff Development to encourage the professional development of staff.
Collaborates with the Clinical Care Coordinator in order to establish and evaluate an appropriate plan of care for the residents.
Collaborates with the rehabilitation team and monitors the provision of restorative nursing care of residents as indicated.
Conducts routine performance appraisals related to job descriptions annually as needed.
Demonstrates commitment to professional development and collaborates.
Demonstrates knowledge of SCNR Compliance Program and reports concerns to management.
Directs, supervises and evaluates nursing care provided to residents
Directs, supervises and evaluates nursing care provided to residents assisting team members when professional judgement nursing skills are needed.
Documents unusual occurrences.
Ensures adequate staffing based on required staffing levels.
Establishes and maintains interdepartmental communication.
Facilitates daily morning report meetings.
Implements the plan of care for the resident based on an assessment and measurable goals established by the comprehensive care plan team.
Initiates and leads team conferences in development of individualized comprehensive care plans.
Maintains clinical expertise through attendance at formal and informal educational programs. Recommends and implements innovative methods to improve the quality of resident care.
Maintains the standards of nursing care and implements the policies and procedures of The Silvercrest Center and the Department of Nursing Services.
Participates in all Unit Personnel’s orientation as directed by the Assistant Director of Nursing Services.
Participates in all Unit Personnel’s orientation as directed by the Associate Director of Nursing
Participates in economical utilization of supplies and ensures that equipment is maintained in a clean and safe manner.
Participates in the implementation and evaluation of facility Performance Improvement.
Participates in the process of disciplinary and evaluator conferences as requested by the Nurse Manager, Director of Nursing, and Associate Director of Nursing.
Participates in the staff development program by attending regular programs and conducts educational presentations when required by Nursing administration.
Participates on committees to develop and update Policies and Procedures.
Serves on nursing and another administrative committee as requested
Supervises the administration of medications and treatments to assigned residents, and administer same when necessary.
Takes appropriate action on deficiencies and evaluates the effectiveness of resident care protocols.
When appropriate, provides care/service taking into consideration the age related physical and psychosocial needs of the resident/patient.
Performs additional responsibilities as delegated by the Director/Associate Director of Nursing.
Performs any and all such duties related to the effective and efficient operation of Silvercrest Center as assigned.
Skills & Requirements:
· Bachelor’s Degree in Nursing is required. Masters Preferred.
· NYS Registered Nurse License
· Minimum 2-5 years of Managerial Experience in a Healthcare facility.
· Minimum 1-2 years LTC and/or Acute Care Management Experience required.
· ANCC Gerontological Certification preferred.
· Must possess the ability to set goals and work in a self-directed fashion.
· Must possess the ability to deal professionally and effectively with employees, residents and the public.
· Must have excellent writing, communication, and computer skills.
