Position Summary
As a Hospice and Home Health CNA, you will provide compassionate, hands-on care to patients in their homes, supporting their physical, emotional, and dignity needs. You will play a vital role on an interdisciplinary care team, serving individuals with chronic conditions, recovery needs, or end-of-life care.
Key Responsibilities
- Provide personal care including bathing, grooming, dressing, and toileting
- Assist with mobility, transfers, and safe ambulation
- Monitor and report changes in patient condition to the clinical team
- Support patients with activities of daily living (ADLs)
- Offer companionship and emotional support to patients and families
- Maintain accurate documentation per agency and regulatory standards
- Follow individualized care plans under supervision of licensed nursing staff
- Adhere to infection control, safety, and hospice care protocols
Qualifications
- Current Oregon Certified Nursing Assistant (CNA) certification
- Previous experience in home health or hospice preferred, but not required
- Ability to work independently and manage time effectively
- Strong communication and interpersonal skills
- Compassionate, patient-centered approach to care
- Valid driver’s license, reliable transportation, and insurance
