Position Summary As a Hospice and Home Health CNA, you will provide compassionate, hands-on care to patients in their homes, supporting their physical, emotional, and dignity needs. You will play a vital role on an interdisciplinary care team, serving individuals with chronic conditions, recovery needs, or end-of-life care. Key Responsibilities Provide personal care including bathing, grooming, dressing, and toileting Assist with mobility, transfers, and safe ambulation Monitor and report changes in patient condition to the clinical team Support patients with activities of daily living (ADLs) Offer companionship and emotional support to patients and families Maintain accurate documentation per agency and regulatory standards Follow individualized care plans under supervision of licensed nursing staff Adhere to infection control, safety, and hospice care protocols Qualifications Current Oregon Certified Nursing Assistant (CNA) certification Previous experience in home health or hospice preferred, but not required Ability to work independently and manage time effectively Strong communication and interpersonal skills Compassionate, patient-centered approach to care Valid driver’s license, reliable transportation, and insurance Our Culture At Signature Healthcare at Home, culture is not just a statement—it is lived every day.
