Registered Nurse (RN) Case Manager– Home Health
Position is Full Time Hourly ~ Monday through Friday~ Occasional Weekends

What You’ll Be Doing (High Level)
- Providing skilled, one-on-one nursing care in patients’ homes
- Educating and supporting patients and caregivers
- Managing medications, symptoms, and complex clinical needs
- Collaborating closely with physicians and care teams
- Supervising LPNs

What You’ll Need
Active Registered Nurse (RN) license
Current BLS certification (AHA)
Valid driver’s license and insured vehicle
Strong clinical judgment and ability to work independently
Leadership mindset with flexibility and initiative
Home Health or Hospice experience preferred (not required)
