What are the responsibilities and job description for the Home Care RN position at Sightspectrum?
Provide individualized, goal-oriented nursing care in accordance with physician orders and the nursing process.
Assess patients’ health conditions and develop, implement, and evaluate care plans.
Manage an assigned caseload and coordinate patient care with the healthcare team.
Serve as Case Manager when nursing is the lead discipline.
Educate patients and families on health management and treatment plans.
Support and mentor a Nurse Resident in clinical practice.
Current, valid VT Registered Nurse (RN) license.
Experience in nursing; home health experience preferred.
Strong patient assessment, communication, and care coordination skills.
Ability to work independently and provide compassionate care in patients’ homes.
Salary : $60 - $70
