Job Summary
We, Sight Spectrum LLC, are seeking an experienced Home Health Registered Nurses. The Home Health Registered Nurse (RN) provides individualized, goal-oriented nursing care in patients' homes in accordance with physician orders and established nursing practices. The RN uses the nursing process to assess patient needs, develop and implement care plans, evaluate patient outcomes, and promote overall health and well-being.
When serving as the lead discipline, the RN also assumes case management responsibilities, coordinating patient care and collaborating with other healthcare professionals to ensure comprehensive and effective service delivery.
Key Responsibilities
- Provide individualized, goal-oriented nursing care in accordance with physician orders and established standards of practice.
- Assess patients' health conditions, identify care needs, and develop appropriate nursing care plans.
- Implement and evaluate nursing interventions to achieve patient-specific goals and improve health outcomes.
- Monitor patient progress and communicate significant changes in condition to the appropriate healthcare providers.
- Serve as the case manager when nursing is the lead discipline, coordinating care and ensuring continuity of services.
- Collaborate with physicians, healthcare professionals, patients, and families to support comprehensive care delivery.
- Promote patient safety, health education, and independence within the home environment.
- Maintain accurate and timely patient documentation in accordance with organizational policies and applicable regulations.
Minimum Qualifications
- Minimum Experience: 1 year of nursing experience required.
- Required Certifications: Basic Life Support (BLS/BCLS).
- Education: Nursing degree from an accredited program.
- Licensure: Active Registered Nurse (RN) license appropriate for the state of practice.
Preferred Qualifications
- Previous home health nursing experience.
- Strong patient assessment, care planning, and case management skills.
- Excellent communication and coordination skills.
- Ability to provide compassionate, individualized care in a home setting.
Pay: $42.00 - $65.00 per hour
