Essential Duties and Responsibilities
- Plan, coordinate, and facilitate ongoing support groups for individuals experiencing life-changing challenges
- Develop and organize workshops focused on emotional wellness, coping strategies, grief support, and social connection
- Provide resource navigation, and referrals to community services
- Collaborate with healthcare providers, community organizations, and internal departments to ensure holistic support for participants
- Maintain accurate and timely documentation in accordance with program standards
- Stay informed about community resources and best practices in support group facilitation and psychosocial care
- Participate in regular clinical and task supervision, case consultation, and team meetings
- Contribute to additional program initiatives and community outreach efforts as needed
Qualifications
- Master’s Degree in Social Work required
- LMSW or LCSW in New York State required
- Interest in facilitating groups and supporting individuals going through life transitions
- Initiative to find and use community resources related to medical support, grief, mental health, and wellness services
- Strong interpersonal and communication skills with a compassionate and professional approach
- Strong organizational skills with the ability to work both independently and collaboratively
- Comfortable using Microsoft Office applications and learning additional software systems as needed

This full-time, on-site role in Roslyn, NY includes conducting assessments, case management, providing counseling, and connecting participants to internal programs and external community resources. The Social Worker will facilitate support groups, maintain accurate documentation, and collaborate with colleagues to ensure a coordinated approach to care. Additional responsibilities include outreach, advocacy, and participation in program development to strengthen the Center’s services and impact.
Qualifications
- Clinical social work skills, including assessment, counseling, and care planning.
- Experience with case management, resource referral, and advocacy within community-based settings.
- Strong communication, active listening, and interpersonal skills for working with diverse populations.
- Ability to maintain thorough, confidential records and manage time effectively within schedule.
- Licensed in Social Work or a related human services field.
- Experience in community centers, nonprofits, or social service agencies is beneficial.
