Job Description
The Outreach Coordinator provides a wide range of clerical support to the Care Management Department and Outpatient Clinic in the areas of data management, medical records/billing, community contact, monthly reporting, and general clerical duties. Responsible for the coordination of the outreach clinics at the hospital and in surrounding geographic area for infant, toddler, preschool, school age, adolescent, and young adult (up to age 21) patients. Duties include, but are not limited to, scheduling and coordinating the outreach clinics including arranging travel accommodations, clinic locations, etc., staffing, patient appointments and follow-up of ordered tests. Works in conjunction with the clerical staff in managing the patient information, preparing for the clinics and staffing the clinics. Participates in the continuous quality management program as it relates to the outreach clinic setting.

Key Responsibilities
- Maintain accurate, current databases using computer equipment and software programs.
- Identify meaningful data for trends analysis.
- Develop and maintain departmental forms.
- Provide clerical support to Care Management and Outpatient Services Departments.
- Responsible, under the direction of the OPC Manager, for coordination of all outreach clinics in their entirety
- 1 year of experience with Medical Terminology.
- 1 year of experience with Electronic Medical Records software.
- 1 year of experience with database management.
- Proficient in Microsoft Office
- High School diploma or GED
- 3 years experience in a healthcare setting.
- Pediatric experience.

Salary : $31 - $47
