SUMMARY:
The Registered Nurse (RN), Case Manager – Comprehensive Homecare (a) provides care to clients needing Homecare services in their home or community setting; (b) provides development, implementation and evaluation of individual client care plans; and (c) maintains an ongoing responsibility for an assigned caseload.
JOB SPECIFICATIONS:
Education, Experience and Credentials
- Graduate of an accredited school of nursing required.
- Must possess and maintain State of Minnesota RN licensure.
- One year or more clinical nursing experience is preferred.
- Med Surge, TCU/LTC, Home Care, Hospice or public health preferred.
- Possess a valid driver's license with good driving record, and proof of valid insurance. Must have reliable transportation.
- CPR Certification required.

Compensation: An individual's pay rate within the posted range may be determined by various factors, including skills, knowledge, relevant education, experience, and market conditions. Additionally, our organization prioritizes pay equity and considers internal team equity when making any offer. Hiring at the maximum of the range is not typical. Shift and/or other differentials may be available to increase your pay rate for certain shifts or work.
