What You'll Do:
Evaluate and treat residents with speech, language, voice, cognitive, and swallowing disorders
Develop individualized treatment plans focused on maximizing independence and quality of life
Collaborate with therapy, nursing, and interdisciplinary teams to provide exceptional resident care
Complete timely and accurate documentation using Net Health
Utilize PointClickCare (PCC) as part of the interdisciplinary care process
Educate residents, families, and caregivers on treatment strategies and recommendations
What We're Looking For:
Current Pennsylvania Speech-Language Pathologist license
Certificate of Clinical Competence (CCC-SLP) required
Previous Skilled Nursing Facility (SNF) and/or Long-Term Care (LTC) experience strongly preferred
Experience with Net Health and PointClickCare (PCC) preferred
Excellent communication, clinical, and teamwork skills
