Role Description
- EMR = DeVero (myUnity)
- Provide skilled physical therapy evaluations and treatment to patients in their homes, in accordance with physician orders and the individualized plan of care
- Conduct initial and periodic OASIS assessments to determine appropriateness for home health services and update the plan of care as needed
- Design and implement treatment plans addressing mobility, strength, balance, gait, and functional independence, with a focus on fall prevention and safe home navigation
- Educate patients and caregivers on home exercise programs, safety precautions, adaptive equipment use, and energy conservation techniques
- Collaborate with the interdisciplinary care team (RNs, OTs, SLPs, MSWs, physicians) to coordinate patient care and communicate changes in condition
Qualifications
- Active, unrestricted California Physical Therapist license or willing to obtain
- Doctor of Physical Therapy (DPT) or equivalent PT degree from an accredited program
- Recommended 1 year of clinical experience; home health experience strongly preferred
- Current CPR/BLS certification
- Valid driver's license, reliable transportation, and current auto insurance
- Ability to travel between patient homes within the assigned service area
- Proficiency with EMR/point-of-care documentation systems
- Strong time management and ability to work independently in a field-based role
- Bilingual candidates a plus, depending on region
