Position Summary 

As a member of the Rehabilitation staff, plans and implements specific programs for individual patients and patient groups according to the principles and practices of occupational therapy. 

Primary Responsibilities 

• Screens all new admissions and any patients referred by the medical team through gross assessment and makes recommendations to the nursing staff
• Plan, develop, organize, implement, evaluate, and direct occupational therapy programs and activities, in accordance with current rules, regulations, and guidelines that govern the long-term care facility.
• Evaluate assigned patients, gathering data from a variety of sources, such a patient records, interviews, observation and team members
• Develop treatment plans for assigned patients including specific goals and activities.  Reevaluate and modify plan at periodic intervals and as necessary. 
• Prepare written documentation as required by the profession and the department, such as evaluation results, individualized treatment plans, progress notes and other written reports as required.
• Communicate observations, facts and comments with interdisciplinary team members, individually and through team meeting presentations.
• Recommend and develop new services and programs.
• Maintain department policies, procedures, objectives, and quality assurance programs.
• Attend meetings and participate on committees as required.
• Enhance professional growth and development through participation in educational programs, current literature, in-service meetings, workshops, etc.
• Provide in-service training specific to occupational therapy to other rehab staff, nursing staff, patients and family members.
• Supervise the COTA , develop treatment plans and goals and provide training as needed.
• Maintains accurate billing information in accordance with Sherrill House procedures on a daily basis
• Maintains confidentiality of all patient, facility and physician related information
• Meet with administration, medical and nursing staff, as well as other related departments in planning therapy services.
• Develop and maintain occupational therapy standards.
• Interpret occupational therapy policies and procedures to personnel, residents, family members, etc., as necessary.
• Participate in discharge planning, development and implementation of resident care plans, resident assessments, etc., as necessary.
• Maintain an excellent working relationship with the medical profession and other health related facilities and organizations.
• Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the department.
• Participate in the development and implementation of resident assessments (MDS) and care plans, including quarterly reviews.
• Assist in developing occupational therapy plans for individual residents in coordination and conjunction with the resident assessment (MDS).

License Requirement

• Must possess a current, unencumbered license to practice as an Occupational Therapist in this state.

Educational Requirements:  A Bachelor's Degree in Occupational Therapy from an accredited college or university.

Experience: A new grad or minimum of 2 years of experience in a therapist capacity in a hospital, skilled nursing care facility, or other related medical facility and training in rehabilitative and restorative therapy practices.

Salary : $38 - $43
