Job Summary: The pharmacist is responsible for execution of established aspects of the pharmaceutical needs of patients in both the inpatient and outpatient setting as developed by Pharmacy Director.  The pharmacist shall be a resource to physicians and nurses in matters dealing with all aspects of drug therapy.  It is also imperative that Pharmacist maintains compliance with all agencies that regulate the use of drugs and their preparation and distribution of medications.

Essential Duties and Responsibilities:

  • It is the responsibility of all employees of Sheridan Community Hospital to promote the Mission Statement, Values, Vision, and Goals of the organization. 
  • Pharmacist is also responsible for assuring that the business practices of their departments are following current state and federal law. 
  • In collaboration with Pharmacy Director he/she assures us that the department is in compliance with current standards established by the AOA.
  • Responsible for implementing and coordinating all policies dealing with pharmaceuticals within the hospital established by Director of Pharmacy.
  • Acts as a leader for Antibiotic Stewardship Program and Antimicrobial Stewardship.
  • Responsible for procurement, storage, preparation, dispensing, recording, and control of all drugs used in the hospital.
  • Coordinates pharmacy policies that affect other departments.
  • Assists in preparing and keeping current the hospital formulary.
  • Maintains adequate supply of medications listed in the hospital formulary.
  • Prepares medications by reviewing and interpreting physician orders and detecting therapeutic incompatibilities.
  • Establishes relationships with other departments to be used as resources for Pharmacy related issues and emergencies. 
  • Trains all personnel assigned to the pharmacy.
  • Is responsible for restocking medications in the night cupboard and crash carts throughout the hospital.
  • Oversees and conducts inspections of all drug storage areas within the hospital for outdated medication on a weekly bases.
  • Maintains current records of all controlled substances.  Is responsible for investigating any discrepancy found during routine counts.
  • Procures all IV solutions and is responsible for proper functioning of pumps used for administration of fluids.
  • Supervises and assists in dispensing of prescriptions to the nursing units.
  • Informs physicians and nursing personnel of any possible untoward reactions to medications or drug interactions.
  • Responsible that admixtures are prepared according to established policy.
  • Is responsible for proper disposal of any hazardous substance that is prepared in the pharmacy.
  • Prepares a policy for handling spills of hazardous or potentially hazardous materials used in the pharmacy.
  • Keeps the medical staff and nursing personnel informed of new trends in drug therapy.
  • Attends educational seminars to keep current on all aspects of pharmacology.
  • Is an active member of the Pharmacy and Therapeutics Committee 
  • Is responsible for implementing decisions of the P T Committee per direction of Pharmacy Director
  • Is responsible for establishing methods for promoting the safe administration and use of medications.
  • Participates in preparation of annual operational and capital budget with strategies and goals for the department, to the hospital administration.
  • When unavailable to the department during off duty hours, designates an alternate to deal with emergency situations.
  • Responds to any complaints by patients or families that relate to pharmacy issues.  Reports any serious problems to the Risk Manager immediately.
  • Maintains up to date pharmacy library.
  • Participates as necessary in the training of pharmacy interns and nursing students
  • Interacts with the community on issues that promote health care concerns and the hospital.
  • Coordinates and provides for adequate coverage of the department during both on duty hours and off duty hours.
  • Participate in quality improvement projects. Attend training and meetings as required
  • Maintains awareness of current quality and safety measures on the unit and follows guidelines or reporting measures to ensure safety of patients, visitors, and staff.
  • Knowledge and skills in quality improvement and research methodologies.

  • Performs other related duties as assigned.

Interdepartmental Functions:

  1. Active participation in hospital committees and task forces.
  2. Active participation in quality management issues.  Submits reports and data as requested by the Quality Management Council.
  3. Demonstrates the ability to use channels of communication to address concerns, problems, and conflicts with other departments.
Qualifications:

Education, Experience and Other Requirements:

  • Bachelor of Science degree in Pharmacy from an accredited College of Pharmacy
  • Licensed by the State of Michigan as a Pharmacist
  • Three years of experience as a pharmacist, preferably as a hospital pharmacist. 
  • Previous managerial or supervisory experience preferred.
  • Excellent written and verbal skills. 
  • Ability to interact with physicians, nurses, and other health care professionals on matters that deal with the pharmaceutical needs of patients.
