Licensed Practical Nurse (LPN) – Hospice Care

Position Summary
Shepherd's Cove Hospice is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our interdisciplinary hospice team. This position primarily provides skilled nursing care to patients in their homes while serving as a backup resource in our Inpatient Unit (IPU), Admissions Department, and contracted facilities as needed. The ideal candidate is committed to providing exceptional end-of-life care and supporting patients and families with compassion, dignity, and respect.
Essential Duties and ResponsibilitiesPrimary Home Care Responsibilities
· Provide skilled nursing care to hospice patients in their homes under the supervision of the Registered Nurse Case Manager.
· Assess and monitor patient conditions, symptoms, and responses to treatment.
· Administer medications and treatments as prescribed and according to the patient's plan of care.
· Educate patients, caregivers, and family members regarding disease processes, symptom management, medications, and comfort measures.
· Document patient care accurately and timely in the electronic medical record.
· Communicate changes in patient condition to the RN Case Manager and interdisciplinary team.
Backup Inpatient Unit (IPU) Responsibilities
· Assist with direct patient care and symptom management in the hospice inpatient setting.
· Collaborate with the interdisciplinary team to ensure quality end-of-life care.
· Monitor and report patient status changes.
Backup Admissions Responsibilities
· Assist with hospice admission assessments and information gathering.
· Educate patients and families regarding hospice services and philosophy.
· Facilitate smooth transitions into hospice care.
Backup Facility Responsibilities
· Provide hospice nursing care to patients residing in skilled nursing facilities, assisted living communities, and long-term care settings.
· Collaborate with facility staff to coordinate care and implement the hospice plan of care.
· Ensure continuity and quality of care across care settings.
Qualifications
· Current Licensed Practical Nurse (LPN) license in the State of Alabama.
· Minimum of one (1) year of nursing experience required.
· Hospice, home health, long-term care, or palliative care experience preferred.
· Current CPR certification.
· Valid driver's license, reliable transportation, and proof of automobile insurance.
· Ability to travel throughout Shepherd's Cove Hospice service area.
· Strong clinical, communication, and organizational skills.
· Ability to work independently and as part of an interdisciplinary team.
