The Discharge Coordinator plays a critical role in ensuring a smooth transition for patients from the hospital to their home or other care settings. The ideal candidate will possess strong knowledge of medical terminology, coding, and discharge planning processes, along with experience in acute care settings.
Duties:
- Validate the information provided upon registration.
- Collect copays.
- Collaborate with multidisciplinary teams including physicians, nurses, social workers, and case managers to facilitate patient transitions and schedule follow up appointments.
- Ensure compliance with HIPAA regulations while managing sensitive patient information.
- Utilize knowledge of CPT coding, ICD-9, and ICD-10 for accurate medical record documentation.
- Maintain accurate medical records and documentation related to discharge processes.
Experience:
- High School Diploma or GED.
- Familiarity with medical coding (CPT, ICD-9, ICD-10) is essential for effective documentation and billing processes.
- Experience in acute care settings is highly desirable.
- Knowledge of medical terminology and the ability to navigate electronic medical records systems is required.
