Role: Case Manager Site Outreach (Chronic Care Support)

About the Role
We are seeking compassionate, organized, and mission-driven Case Managers to support seniors enrolled in our federally funded wellness and Chronic Care Management (CCM) program. This role combines remote care coordination with scheduled outreach visits to sites such as senior centers and affiliated clinics.
You’ll help patients navigate wellness resources, conduct structured check-ins, and document services in alignment with CMS guidelines. This is a flexible, field-based opportunity ideal for professionals who want to make a measurable impact in underserved communities.
In addition to assigned outreach sites, Case Managers may also enroll eligible patients from their own affiliated networks—such as medical centers, home care agencies, adult day programs, senior care facilities, or retirement clubs. This flexibility allows you to leverage existing relationships and community trust to expand access and impact.
Responsibilities
Site-Based Outreach
- Visit assigned sites or community clinics
- Conduct wellness check-ins (vital signs) and share program materials
- Provide culturally sensitive support and reassurance
- Collect and enter vital signs (BP, glucose, weight)
- Complete Google intake forms and first notes in CureMD
- Verify insurance and basic health info
- Share approved health education flyers, gifts, and highlight key points in the session based on the topic of the day (e.g. lifestyle tips, disease awareness)
Community-Based Enrollment
- Identify and enroll eligible patients from your own network (e.g. affiliated clinics, home care, senior centers, retirement clubs)
- Use approved scripts and materials to introduce the program and complete documentation
- Ensure all enrollments meet CMS documentation standards and are entered in CureMD
Compensation:
Site Visit Payment – $50 per completed site visit (Average 2 hours)
– Target: ~40 visits/month = $2,000/month
One-Time Enrollment Support Payment
– For every new eligible patient you fully enroll (Google form vitals CureMD note), you’re credited toward case support
– You earn $30 care coordiation fee for every 10 new eligible patients enrolled that month
– Approx. 450 new patients/month = 45 hours × $30 = $1,350
Estimated Monthly Total: $2,000 $1,350 = $3,350

We are actively placing Case Managers in:
- Bronx/Brooklyn: High-density housing sites with multilingual patient panels
- Manhattan: Senior centers and wellness hubs in Upper Manhattan and Harlem
- Hempstead: Suburban outreach with strong community partnerships
- Hicksville: Clinics and housing sites with diverse patient needs

You may select one or multiple regions based on availability.
Qualifications
Required
- Medical Assistant certification
- 1 year experience in outreach, case coordination, social work, or patient engagement
- Strong communication and documentation skills
- Ability to work independently and manage time effectively
Preferred
- Associate’s or bachelor’s degree in public health, social work, or human services
- Experience with Medicare populations or CMS-aligned programs
- Familiarity with HIPAA, consent workflows, or care navigation
- Multilingual fluency
