Description
- Overall performance and implementation of Teen Reach programming
- Assists Teen REACH Program Coordinator by formulating and coordinating individual needs assessments, goal setting, face-to-face and phone conversation on a weekly/bi-weekly basis, family engagement, linkage with community agencies, and follow-up services for youth and participants as needed;
- Works to assist youth to determine what factors put them at risk of engaging in or becoming victims of aggressive behavior and other anxieties;
- Helps to determine what methods each youth should use in order to avoid risk- taking factors and reach their goals;
- Reaches out and re-engage youth that have inconsistent attendance, work with families to bring resources to the home, and connect with programs;
- Conducts data collection and coordinate reporting to funders, marketing staff, and other stakeholders;
- Utilizes the Illinois Department of Human Services (IDHS) required eCornerstone internal data system to ensure program data are properly entered and exported for program and administrative need
- Provides support and program leadership for youth ages 11-17 who participate in Teen REACH services;
- Other duties as assigned.
Education:
- Associate’s Degree in social work, criminal justice, related field, or equivalent work experience, preferred.
- Experience working with teens and young adults, preferred
- Experience working in neighborhoods on southwest sides.
- Experience working with individuals dealing with factors that put them at-risk of becoming victims of aggressive behavior.
- Experience working one on one with youth involved in the juvenile justice system.
- Evening and weekend availability required.
- Sensitivity to cultural diversity is required. Ability to communicate and cooperate with diverse families, various professionals and community groups.
- Must be able to work independently.
- Must be able to remain stationary 50% of the time.
- Must be able to frequently move about inside the office to access file cabinets, office machinery, etc.
- Occasionally ascends/descends stairs.
- At times may be required to lift up to 25 lbs. for various classrooms and events needs.
- Frequently moves in outdoor weather conditions.
