As an LPN, you’ll help ensure individuals receive safe, high-quality care while supporting their health, independence, and overall well-being. You’ll work closely with residential staff, families, physicians, and interdisciplinary teams to coordinate care and promote positive health outcomes.

What You’ll Do
- Provide nursing oversight and monitor individuals' health status, medications, and treatments.
- Support the implementation of individualized health care plans and physician orders.
- Train and provide clinical guidance to direct support and residential staff.
- Monitor changes in health and communicate concerns to RNs, physicians, and the interdisciplinary team.
- Maintain accurate medical documentation, medication records, and nursing notes.
- Participate in interdisciplinary meetings, care coordination, and quality improvement activities.
- Promote infection prevention, medication safety, and compliance with applicable regulations.
- Participate in the Nurse On-Call rotation, including occasional evenings, weekends, and holidays.
- Provide clinical support and guidance to residential programs and assist with day program coverage as needed.
