Role Description
Seva Care Coordinator is a hybrid administrative and field-based role responsible for supporting family caregivers and clients within the Seva program. This position serves as a key connection between caregivers, clients, nursing leadership, and internal operations to help ensure high-quality care, timely documentation, and ongoing compliance with Georgia Medicaid waiver requirements.
The Care Coordinator provides direct support to caregivers through regular communication, in-home visits, education, and guidance while also assisting with client onboarding, electronic documentation review, compliance tracking, and care coordination activities. This role requires strong organization, professionalism, relationship-building skills, and the ability to manage both field responsibilities and administrative tasks in a fast-paced healthcare environment.
The ideal candidate is comfortable working independently in the community, communicating with families, navigating electronic healthcare systems, and maintaining accurate records while helping ensure clients continue to receive uninterrupted services.
Qualifications
* 2 years of experience in healthcare, home care, case management, caregiving support, healthcare administration, or community-based services preferred
* Experience working with Medicaid waiver programs such as SFC, CCSP, or SOURCE is highly preferred
* Comfortable conducting regular in-home client and caregiver visits throughout the service area
* Strong communication and relationship-building skills with caregivers, clients, and internal staff
* Ability to manage multiple tasks, deadlines, and documentation requirements in a fast-paced environment
* Strong organizational and problem-solving skills with attention to detail
* Experience using electronic health records (EHR), EVV systems, or healthcare documentation platforms preferred
* Comfortable reviewing electronic caregiver logs, documentation, and compliance-related tasks
* Ability to work independently while maintaining strong communication with the internal team
* Basic computer proficiency including email, Microsoft Office, Google Workspace, and healthcare software systems
* Valid driver’s license, reliable transportation, and ability to travel locally for field visits
* CNA, Medical Assistant, Health Coach, Community Health Worker, or similar thealthcare experience is a plus
